Job Description

Position Overview: Is responsible for the coordination and final review of the MDS and Care Planning Process, Utilization review, Quality Improvement studies and the Resident Tuberculosis Program.

Essential Functions and Responsibilities:

    • Responsible for conducting and coordinating the development and completion of Resident assessment (MDS) in accordance with current rules, regulations, and guidelines including the implementation of the RAPs/CAA and Triggers/CAT’s.
    • Coordinates and provides oversight for the RAI processes including completion of documentation as necessary to ensure that deadlines are met by the interdisciplinary team.
    • Ensures that resident assessments are completed within appropriate time frame of the resident’s admission and is submitted timely.
    • Work with insurance companies to obtain certification for residents stay including completing documentation and submitting information accurately and timely in order to obtain coverage for treatment.
  • Serves as a member of the Nursing Management team and the On-call schedule/early morning staffing rotation.

Demonstrated ability to write NCP-MDS, perform utilization review, experience in Quality Improvement methodology.

Education and/or Experience:  Graduate of an Accredited School of Nursing or a graduate of an approved RN/LPN program.  Two year experience in Long Term Care preferred.  Must be knowledgeable of Nursing and Medical practices and procedures, as well as laws, regulations, and guidelines that pertain to Long- term Care. Previous experience/training in Rehabilitation and Restorative Nursing practices.

Supervisory Responsibilities:  Supervisory responsibilities for the MDS staff  in the absence of the MDS Manager.

Language Skills:  Ability to read write and speak in English.

License, Certificates, Registration: Licensed in the State of Michigan as a LPN or RN.  Must possess and maintain current CPR certification. Must be AANAC Certified or obtain certification within 6 months of hire.
