Job Description

Position Overview: Is responsible for the coordination and final review of the MDS and Care Planning Process along with the Interdisciplinary team in accordance with State and Federal regulations, utilization review, and selected Quality Improvement initiatives.

Essential Functions and Responsibilities: include the following. Other duties may be assigned.

Leadership

  1. Demonstrates ability to coordinate and educate staff on MDS process.
  2. Ability to identify issues related to MDS processes and effectively communicates to DON and other members of Nursing Leadership Team.
  3. Responsible for managing, leading and directing the MDS staff.
  4. Serves as the chairperson of the Interdisciplinary Care Plan team.
  5. Responsible for creating a MDS Assessment calendar.
  6. Ensures that MDS assessments are completed in the required time frame by members of the Interdisciplinary Team.
  7. Monitors MDS compliance process by the Interdisciplinary Team.
  8. Ensures that the information on the MDS is accurate and timely.
  9. Responsible for auditing MDS documentation to ensure accurate and timely submissions.

Human Resources

  1. Participation and input into hiring and performance review processes of MDS staff.

Business Acumen/Financial

  1. Demonstrated understanding of MDS coding and RUGs levels and how they relate to reimbursement.

Personal Relations

  1. Displays control and composure in complex, stressful or ambiguous situations, anticipates problems and plans accordingly, uses plan effectively to manage situation.
  2. Interacts professionally with all levels of staff.

Customer Service

  1. Interface with other departments and services to facilitate the delivery of resident care.

Department Specific

  1. In-depth knowledge of MDS processes, including but not limited to:
    1. MDS
    2. RAPs
    3. Care Planning
    4. Utilization Review
    5. PDPM
  2. Successful completion of AANAC MDS Coordinator Certification course within six months of hire.
  3. Member of the Nursing Leadership Team.

Demonstrated ability to write NCP-MDS, perform utilization review, experience Quality Improvement methodology.

Education and/or Experience:  Graduate of an Accredited School of Nursing.  Two years of experience in Long Term Care. 1-2 years of recent MDS experience.

Supervisory Responsibilities:  Responsible for managing the MDS team.

Language Skills:  Ability to read, write and speak in English. Excellent verbal and written  communication skills required. 

License, Certificates, Registration: Current license as a Registered Nurse in Michigan.  AANAC MDS Coordinator Certification or successful completion within six months of hire.
